Frozen Shoulder Risk: Why Diabetes, Thyroid Disorders and Midlife Hormonal Changes Are Raising Concern
New research identifies diabetes as one of the strongest risk factors for frozen shoulder, while experts say early diagnosis and treatment can significantly improve recovery.
Frozen shoulder, medically known as adhesive capsulitis, is gaining renewed attention after recent research highlighted several health conditions that can sharply increase the likelihood of developing the painful joint disorder. The condition causes progressive shoulder pain, stiffness and a severe reduction in movement, often interfering with routine activities such as dressing, driving and sleeping.
A 2026 systematic review and meta-analysis found that people with diabetes face nearly a fourfold higher risk of developing frozen shoulder compared with those without the disease. Researchers also identified poor blood sugar control as a major contributor, suggesting that effective diabetes management may help reduce the risk.
The review further identified obesity, high cholesterol, hypertension, thyroid disorders, smoking and alcohol use as additional risk factors. Adults between the ages of 40 and 65, particularly women, were found to be more susceptible, reflecting the condition’s strong association with metabolic and hormonal changes.
Doctors are also investigating the possible role of menopause in frozen shoulder. Emerging evidence suggests that declining estrogen levels during perimenopause and menopause may affect connective tissue elasticity and increase inflammation around the shoulder joint. Ongoing clinical studies are examining whether hormone-related therapies could improve outcomes in selected patients, although definitive evidence is still awaited.
Frozen shoulder typically develops in three stages. The first, known as the “freezing” stage, is marked by increasing pain and gradually worsening movement. During the “frozen” stage, stiffness becomes more pronounced even as pain may lessen. Finally, the “thawing” stage brings a slow recovery of shoulder mobility, though the entire process can take months or even several years.
Medical experts emphasize that early recognition is critical. Conservative treatment—including pain-relieving medications, corticosteroid injections when appropriate, and structured physiotherapy—can significantly reduce symptoms and improve shoulder function. Recent evidence indicates that non-surgical treatment remains effective for most patients, with substantial improvements in pain and range of motion over time.
Health professionals advise individuals with diabetes, thyroid disease or persistent shoulder pain lasting more than a few weeks not to ignore symptoms. Prompt medical evaluation, regular shoulder mobility exercises and effective management of underlying health conditions can improve recovery and help prevent long-term disability.
